2% Donation

Dear parents,
We sincerely appreciate your help last year in donating a part of your income tax to our Sarunas Marciulionis Basketball Academy. We also ask you to be this kind this year, as well. All the donations will be wisely spent on behalf of your children's physical and spiritual strength, healthy lifestyle education and development of their profesional skills.

You can donate your funds by filling out the application form FR0512 and submiting it to the State Tax Inspectorate (STI) untill the 1st of May, 2011. (Adress: Ulonu street 2, phone: 1882, (8 5) 261 6635). The form has to be submitted in one of several ways: just presented to the STI emploee, sent by mail, or via email, under the VMI instructions. You have to fill out the boxes 1,2,3,4,5,6,7,10,11,12 and 16 and sign it.
